Phantasmal Image copies exactly what was printed on the original creature (unless that creature is copying something else or is a token; see below) and it gains the Illusion creature type and the triggered ability.
Can phantasmal images equip?
Yes for equipment and for when an enchantment is cast targeting Phantasmal Image. However, if you were to have an aura enter the battlefield without beig cast (for example, having Phantasmal Image attack alone with Sovereigns of Lost Alara), it will not target and Phantasmal Image will not be sacrificed.
Can phantasmal image copy a legendary creature?
While Phantasmal Image is not an ‘exact’ copy of the legendary creature, it does have the same name. Therefore, the legend rule applies. This means that all but one of the legendary creatures under your control with that name will die, but you get to choose which one survives.
Is phantasmal image an ETB?
Phantasmal Image is not a token in this case. Any enters-the-battlefield abilities of the copied creature will trigger when Phantasmal Image enters the battlefield.

Yes, the legend rule counts as dying. Mostly. If a permanent is sent to the graveyard from the battlefield for any reason, it counts as dying. However, if a card like Rest in Peace is in play, it won’t count as dying since the permanent never reaches the graveyard in the first place.

Sakashima the Impostor, Olag, Ludevic’s Hubris, Lazav, Dimir Mastermind and Lazav, the Multifarious can copy another legendary creature but keep their names, thus a player can have two legendary creatures with the same functionality.
