The CSU Health and Dental Plan is offered to the undergraduate student body at Concordia University. If you are enrolled in more than three credits per semester and are paying tuition fees at the Canadian or Quebec rates, you will automatically receive coverage.

How do I obtain a letter of attestation from Concordia?
If you are required to demonstrate proof of full-time studies at Concordia to maintain your coverage you can obtain a letter of attestation from the Birk’s Student Service Centre by email. Please note that there is a $20 fee to obtain this letter.
