What is SGHL shoulder?
The SGHL is a focally thickened band in the glenohumeral joint capsule. It originates from the supraglenoid tubercle anterior to the LHBT attachment, and inserts into the fovea capitis line superior to the lesser tuberosity of the bicipital sulcus (13, 15).
What is Buford complex shoulder?
A Buford complex, found in 1.5% of individuals, is the absence of the anterior superior labrum in conjunction with a thickened cord-like middle glenohumeral ligament. The thick middle glenohumeral ligament attaches directly to the anterosuperior glenoid.
How do you fix Buford complex?
Research into the Buford complex and the potential dilemma it poses on athletes has not been studied. As it is a variant on anatomy, the only possible treatment option if one was needed would be to surgically correct the defect through augmentation of the cord like MGHL.
What movement does the glenohumeral ligament prevent?
The superior glenohumeral ligament extends from the supraglenoid tubercle of scapula to the proximal aspect of the lesser tubercle of humerus. Along with the coracohumeral ligament, it supports the rotator interval and prevents inferior translation of the humeral head, particularly during shoulder adduction.

The major movements at the glenohumeral joint are:
- Abduction: upward lateral movement of humerus out to the side, away from the body, in the plane of the scapula.
- Adduction: downward movement of humerus medially toward the body from abduction, in the plane of the scapula.

The MGHL can share a common origin with the superior glenohumeral ligament, can be absent in approximately one tenth to one third of shoulders, and functions to help stabilize the shoulder anteriorly from 0-45 degrees of abduction and external rotation. 5,7,20
What does mghl look like?
The MGHL arises most commonly from the antero superior labrum or neck of glenoid. It can be bifid. Its best seen on axial scans. Look for a linear structure ( blue arrow) running anterior to the labrum ( pink arrow) which eventually inserts into the anterior capsule ( yellow arrows) posterior to subscapularis.
What does the middle glenohumeral ligament (mghl) do?
The middle glenohumeral ligament (MGHL) typically contributes partially to the anterior stability of the shoulder. In a very limited number of cases, the MGHL can cause abrasion on the upper edge of the subscapularis causing persistent pain symptoms for patients.
